COVID vaccine clinic at Wheeling High School

CCSD21 is pleased to host another vaccination clinic in partnership with Jewel-Osco and District 214. This vaccination opportunity is open to any member of our community who will be 12 or older on Wednesday, August 11.

The first dose of the Pfizer vaccine will be administered on Wednesday, August 11. The second dose will be administered on Wednesday, September 1. The clinic will be held at Wheeling High School (900 S. Elmhurst Rd., Wheeling). Appointments are available from 4:00-6:45 PM.

Step 1: Click here to make an appointment. The September 1 second dose appointment will automatically be scheduled for the same time as the August 11 first dose appointment. Please note you will need to make a separate appointment for each individual who will receive the vaccine.

Step 2: Attend both vaccine appointments at the scheduled time: Wednesday, August 11, and Wednesday, September 1.

Please bring the following to both appointments for each person receiving the vaccine:

  • Completed Jewel-Osco consent form (en Español). A blank form will also be emailed to you with your confirmation from Jewel-Osco and extra copies will be available at Wheeling High School. For minors under the age of 18, this must be completed by a parent/legal guardian. Please sign and date on the line provided AND print the adult’s name and relationship to the minor (even though there isn’t a specific line to add this information).
  • Insurance card, if applicable. There is NO COST to you for the vaccine and insurance coverage is not required. However, insurance companies reimburse Jewel-Osco for hosting community events like these if insurance coverage applies.
